Family Registration Form
You may download the pdf file here. Once completed you may do one of the following:
- Print it and drop it off at the library help desk.
- You can also fill out a registration form at the library and give it to the help desk volunteer.
- Mail it to Nora Timmons at the Placitas Community Library, 453 Hwy. 165, Placitas, NM 87043.
- Attach It to an email and send it to nntimmons @ comcast.net (no spaces).
